This was a tire explosion not a blow out. Search for "tire explosion Radomiro Tomic 2000" and you can see that the root cause was a piece of wood left inside the tire.
Hope the driver is ok
Driver was killed. I used to work with the gentleman who investigated the incident. Tyre was being run dry and a piece of wood, with a nail still inside struck the rim causing a spark. The rest is as you see here.
Can that photo be used to promote further safety in mining industry
