I'm impressed at the amount of damage here. The truck must have been doing some speed. Sent it by a reader.

This happens when an operator has a micro sleep while operating a fully loaded Cat 793c at Norwich Park Mine Site, lucky for the operator swerved right other wise the operator would have gone over the high wall
First comment is wrong, the operator never swerved right at all, he went straight ahead through the bund. The only luck he had was where he went to sleep. He was asked about his fatigue level only 15-20 minutes earlier, said he was fine and continued to operate ... I know personally from working here, that Downer fully supported and encouraged tired operators to pull up, they would even swap you out on circuit and drive you to the crib hut for a coffee if you were tired (personal experience) ... this operator ignored all the warning signs and endangered himself and his workmates by stupidly choosing to continue driving tired. I heard he was sacked but I’m not 100% sure. This truck was repaired and put back to work

It was a 793F and no he didn't swerve at all, drove straight ahead through the bund. This truck has since been repaired and is back to work.
This one happened at Norwich Park Mine in Qld Australia. Derek Muirhead was the HSE Superintendent for this site for Downer would have all the info.

Happened at Blackwater, driver fell asleep, woke up in air after launching over bund, then landed nose first. Truck repaired, not written off
He was sacked a few weeks later, but only because he started bragging about the damage he did to the truck
Didn’t happen at Blackwater Mine, it was Norwich Park. Ramp 8. I was on days off when it happened.
